Dress Wise!


Dress- Wise
Oh, what a day, I pondered as I meandered the long corridor to the parking lot! I did not realize that my excitement was just beginning when I heard the fascinating ring tone of my captivating wife. She lovingly enquired about my day and touchingly added a chore without delay. "Hey, don't forget to buy a nice dress for Gomini. It is her birthday, and we are going to surprise her." Sensing possible excuse of a long day, she hit me with a sentiment, "she truly believes you are her brother." It was the moment when truth felt feeble in the face of the emotional upheaval! She concluded her charming conversation that she would text me the size and reminded me of my dexterity in dress selection. The gentle reminder of my finesse in figuring out the fabulous fabric facilitated me to forget my fatigue and browse at Belk.
The dresses were abundant, designs redundant, I was reluctant for a consultant, and yet, I was triumphant. The lady at the dress section after her inspection, commended my selection. After reaffirmation, I whistled my way to home. My attempt to show off as the doyen of dress selection was dotingly declined for lack of time. Savoring her unwavering confidence, I quickly freshened, and we drove off to Gomini's house.
As we alighted, Gomini was delighted. We hugged her and wished her a great birthday. After the exchange of pleasantries, we jovially asked what Madhav (Gomini's husband) bought for her. She very proudly displayed the gold necklace he bought with gleaming eyes. Marveling at Madhav's spousal love, Radha gently nudged me to show her the gift we purchased. Taking stalk of the situation, without much hesitation, shortening the anticipation, I began the demonstration of the culmination of my imagination.
"OMG! It's lovely! So pretty! OMG," Gomini exclaimed in sheer exhilaration. As she began to reassess the dress, tears began to express, and distress became difficult to suppress. She became emotional! We both moved close to her, and I blurted, "it's a small gift, and we are happy it complements Madhav's beautiful necklace." Gomini looked at me, and I saw a tinge of melancholy in her eyes. Her sadness vanquished my gladness. She said, "bhayya (oh brother), do I look so fat to you! I never imagined you had that impression!" It was my time to cringe and singe. How did my assignment turn into my arraignment! Radha immediately grabbed the dress, and the size startled her! She was bewildered to find it big-way big! She stared at me and the expression needed no sub captions to declare how I repeatedly screw up simple tasks!
To pacify Gomini, I said that I righteously followed her bhabi's (sister-in-law) directions. I affirmed that I never question Radha's orders but only obey them. Do or die, and never ask why! Hence, it was not my imagination of her body image but strictly sticking to the specifications. My pride as a dress connoisseur expeditiously evaporated. Radha and I took our phones out to show the text. Then Radha started laughing. Radha apparently typed 8 petite. Her iPhone had its own mind, corrected it to 8 prettier. I was positively convinced to buy a pretty dress at size 8 rather than 8p.
Gomini quickly comprehended the confusion and burst out laughing, hugging both of us. After discerning the smartness of the smartphones, embracing what we read and what it means can make a mighty difference, and how autocorrection may lead to dejection and misperception, with that introspection, I realized the conception of the gift cards.

You may not be dress-wise, but never be size-unwise to avoid surprises that may arise and that's my advice!

Can we learn from this Nature?
Treat Nature as our teacher!
To do a bit more
Expecting nothing more.
If that could become our nature
Peace and joy will be our daily feature!

What Man Proposes Bank Disposes With Bonus



Whether you are married or not, it doesn’t matter. You just need money. That’s where the banks come into play. The banks are very generous; Of course, with your money. Once (decades ago) I wrote a check for $970, and my bank seemed very liberal in the payout. They extrapolated their collective view that $0 in your account has no value and added that 0 to my $970 and issued $9,700 to the already rich landlord company. Since my account did not have sufficient balance to meet their generosity, they kindly froze my account and imposed overdraft fees.

As if this is not enough of a headache for my bald head, my cellphone check that also arrived that day for clearance bounced for lack of funds. The carrier was not happier as my bank set up a barrier for me to pay the dime on time and hence put me off-line. To add salt to the injuries, my credit card check for $980 also bounced for the same reason. The credit card company tried to reach me; I believe, but alas my cellphone had no service. If the credit (card) cannot debit (your account) it will edit your score and discredit your card. I was at a gas station, and my credit card was denied. I had $11 in my wallet, which was enough to buy some chocolate but won’t be enough for filling the tank. I went into the gas station to use the ATM to withdraw, and I had the shock of my life as I had no funds, and they digitally informed that I couldn’t withdraw even one-digit currency! I was 40 miles away to buy furniture at an outlet shop on sale.

I could hardly buy 2 gallons of gas, due to my credit impasse. As a fellow in neonatology, I rarely believed in astrology but felt I should have checked out numerology at least. Moreover, It was unclear how my dear $8,000 could queerly disappear. After I reached home, I realized that cellphone service was turned off rather than lacked the signal. As it was a weekend, the bank was closed in the afternoon, and I couldn’t contact them until Monday. I had to spend the rest of the weekend trying to comprehend and talking to a friend as to what he would recommend!

When I finally reached my bank, they graciously informed me that the dot after 970 was not distinct, and hence the confusion. I asked them why did they not give 97,000 instead of being miserly with $9700 if the dot was indistinct! The teller felt terribly sorry for underpaying the landlord and called the branch manager, who read the amount written in words on the check for a long time as if English was his fifth language and placed his sympathetic hand on my shoulder to announce poignantly that the landlord should return my money. When I cornered him that it was their fault, and they should put back money in my account, he looked clueless and gently asked me from which account I was asking him to transfer the money! It appeared the bank’s generosity towards the business customers equals their atrocity towards the individual customers! After I threatened that I would inform the FBI about this bank fraud, the general manager rushed to sort things out.

It took me ten days to get things straightened out. However, in the meantime, my credit rating took a hit for failing to pay the bills on time. The bank which sank my account to blank, and shrank my credit score to tank, sent a note to thank and to rank their service as swank. I thought it was a prank!

Sex Differences in Nutrition, Growth, and Metabolism in Preterm Infants

Biological differences between the sexes are apparent even from the early part of the pregnancy. The crown-rump length is larger in male fetuses compared to females in the first trimester. Placentae of male and female fetuses have different protein and gene expressions, especially in adverse conditions. Even within the intrauterine milieu, the same extracellular micro RNA may show upregulation in females and downregulation in male fetuses. There appears to be a natural survival advantage for females. Maternal glucocorticoids (GC) play an important role in fetal growth and organ maturation. However, excess glucocorticoids can not only affect growth but the response may be sex-specific and probably mediated through glucocorticoid receptors (GR) in the placenta. Mild pre-eclampsia and asthma are associated with normal growth pattern in males, but in female fetuses, they are associated with a slowing of growth rate without causing IUGR probably as an adaptive response for future adverse events. Thus, female fetuses survive while male fetuses exhibit IUGR, preterm delivery and even death in the face of another adverse event. It is thought that the maternal diet may not influence growth but may influence the programming for adult disease. There is growing evidence that maternal pre-pregnancy overweight or obesity status is directly associated with a higher risk of obesity in a male child, but not in a female child, at 1 year of age. It is observed that exposure to gestational diabetes is a risk factor for childhood overweight in boys but not in girls. It is fascinating that male and female fetuses respond differently to the same intrauterine environment, and this suggests a fundamental biological variation most likely at the cellular and molecular level.

Which one should we use- Birth Weight or Current Daily Weight?

Many use birth weight for calculations of protein, calories, and fluids in preterm infants. However, frequently current daily weight is also used for such calculations. Until now, there was no evidence which strategy is better. We now have evidence to sort out this debate.

The Journey

It began as an inner awakening journey. My spirits woke up for sure; however, the rising temperatures prolonged my spiritual slumber; I thought. Nevertheless, the journey continued. Later, I was asked to awaken the academic interest in the young physician minds. However, the job seemed more like awakening the human lumber from their post-prandial slumber. Even a lance of jokes couldn’t prance them from their trance. Their eyes just would either dance or glance if questioned. Even Devayani (a beautiful dancer) of France wouldn’t have a chance to shake their stance to be in a trance.

Having learned from this resolute group that one should never waver once the decision is made, I began my homecoming journey. As we walked to the check-in counter, my eyes caught the eye-popping flier, which read “ask how you can get $320 with this flight." I hurriedly acknowledged the airline representative greetings and very quickly asked her how I can get that $320. She informed me that by going in world traveler plus instead of the club world as they were overbooked. It was a wallet awakening moment as my financial spirits fired up blazingly. But, my wise wife explained that we paid $2000 for club world and would get meager $320 by downgrading. I had to admit that she was a great fire extinguisher!
With spirits devoid of delight, I dragged my carry-on to board the flight. I just felt like my young audience in the sleepy ambiance and wanted to have the same experience. I summoned a bright looking flight attendant and asked for a pillow. She gracefully bowed and started pressing buttons on my seat. I was bemused. I never knew of a technology that would give me a cushion on the flight with a press of a button! She looked at me equally bewildered as no pillow came from the TV screen! She then dolefully apologized and peacefully left. I was astounded I didn’t know what else to do and began to scratch my shiny bald head. I couldn’t understand how a TV screen would give out a pillow and it was not a 3D printer! I was concerned to ask another smiley attendant, lest she might be a terrifying descendent of the previous mystifying attendant. Sensing my plight on the flight my wife gave me an extra pillow of hers.

The button-fond flight attendant gave me a gentle shake, and I jumped up as if sensing an earthquake. It was time for a flight change; I believe. The airline agent at Heathrow was kind enough to inform us to go one floor up and take the C gate. We didn’t question his suggestion and successfully navigated the airport congestion to the next level. Our faces brightened as we saw the bright C gate sign and there was no queue at all. However, the security personnel standing under the C-gate sign told us firmly that we should go four floors down to catch our flight. We were perplexed as departure flight information indicated we should go to C gate 62. She was unrelenting and insisted with a frown that we should go four floors down. About 20 of us, including an old lady, hurried to the misguided floor only to find out that it was for the train connecting A gates. I wondered how the word misguides was coined! Maybe a Ms. (Miss) like this guided Dr. Samuel Johnson, and he coined the word “misguide”! We were visibly upset and ran four floors up as our departure time was fast approaching.

The misguide was still staffing the C gate allowing five other passengers to pass through. The misguide decided to abide by her previous guidance when she saw us. This time to convince us she went off the floor to discuss with the suppositious superior and sternly asked us to go one level above. Pointing to the flight board information didn’t shake her decision foundation. She wanted to stick to Samuel Johnson’s definition; I suppose. The 20 of us reluctantly shook our heads redundantly and persistently and urgently went upstairs. Everyone had fumes from their ears as we only saw the brooms and the rest-rooms. This time when went down to her, it was her turn to shake the head consistently, probably triumphantly and announced that we could also go through the C gates. By now, we had 20 more people ahead of us. Since it was security personnel, we all behaved like ardent followers of Gautam Buddha and went through the security practicing deep-breathing exercises and counting 1-100 and discussing how the non-violence brought India the independence and so on.

As we ran to the check-in counter chanting and panting, we heard an enchanting announcement that they selectively picked few fortunate customers and called out my name first! I was joyous anticipating a first-class upgrade for my last flight. As my better-half began to accompany me, the agent said the call was just for me. Before I could explain that she was my wife, the security agents took me to a room for a thorough exam and body scan. I looked at my wife, and she had the expression-don’t worry; you go; I have the house keys. I was deadpan in front of the guardsman and in need of a bedpan as it was not the plan in my lifespan!

Does Early Intervention Programs Help Prevent Motor and Cognitive Impairment In Preterm Infants Post-Discharge From NICU?

It is clear that preterm infants are at increased risk for neurodevelopmental delays compared to infants born at term1. Therefore, a number of developmental intervention programs have been introduced in the neonatal intensive care unit setting to help the extremely low birth weight (<1000 grams birth weight) to improve the functional outcomes. Many studies are since then conducted to evaluate the long-term benefits from these intervention programs.
Cochrane neonatal group (a premier evidence-based research organization of the country) recently evaluated the utility of these programs in a meta-analysis. The group appraised 25 studies published until 2015 on this topic and selected 12 studies after applying stringent criteria for bias, randomization, and appropriate allocation. The team published their results in November 2015.
Authors’ conclusions are as follows: “Early intervention programs for preterm infants have a positive influence on cognitive and motor outcomes during infancy, with cognitive benefits persisting into preschool age. With regards to motor outcomes, a meta-analysis of 12 studies showed a significant effect in favor of early developmental interventions at infancy only. The early developmental intervention programs described in this review had to begin within the first 12 months of life, had to focus on the parent-infant relationship and/or infant development and although they could begin while the baby was still in the hospital, had to include a component that was delivered post-discharge from the hospital." Though no effect was shown on cerebral palsy, the motor improvement was, however, noted.
They concluded that further research is needed to determine which early developmental interventions are most effective in improving cognitive and motor outcomes, and to discern the longer-term effects of these programs.
It is quite encouraging that these programs do have a positive impact on the outcomes in preterm infants. We hope more and more parents of high-risk preterm infants take advantage of these early developmental intervention programs provided.

Ethereal Brew

Ethereal Brew
It has been 33 years since I have seen many of my friends from medical school. That thought brought a combination of elation and hesitation. One part of me was delighted, and the other trepidatious. How will I be received? Will I be disheartened or cheered? With the understanding that all of us are matured, and prosperous, I became more optimistic. The journey to San Diego began along the Pacific Coast Highway.
The route was so breathtaking that we could not bear forsaking. Mesmerized by the bewitching beach, my better-half walked down the pebbly path to quench her desire to drench in the ocean waves. She wandered towards the rocky cliffs, ignoring the signs of rip currents. I had no desire to wet even my feet, let alone my seat. As my spouse gleefully drifted towards the perilous Pacific risking the rip tides, I rushed behind her screaming her to halt. Suddenly, it seemed the ocean had a different notion and made a rapid motion with full emotion to abruptly stop my locomotion. It just didn't wet my feet but gave my torso, a salty treat, coercing me to join her. I held on to the blocks of rocks politely declining the active solicitation. The Pacific pacified later. Soaked in salt water, I glanced at my wife; she looked arid, and I felt horrid. Oblivious to the happening, reveling in Pacific roar, sultana turned around with a stunning smile swaying over her scintillating face, innocently inquiring, why am I wet? She was calm, but I needed a balm. She looked serene, and I needed dry-clean.

I had to abandon my shoes and drive bare feet. As we reached the reunion destination, I could feel the palpitation from excitation, as our congregation is going to end the long duration of separation. Everyone was still recognizable and delightful despite donning different shapes. An atmosphere of childish excitement prevailed.
As friends began to open up, lives began to unveil; we felt sad at what some of our buddies had to go through. In the end, they didn’t become casualties to the calamities but instead became champions of the conflicts; bricks the life threw at them. We admired their resilience. Though we were not part of their tribulations; we happily partook in their adulation. Everyone excelled in their fields.
Some went beyond the call of their duty and served in dangerous areas. Others offered voluntary services to uplift the downtrodden.
We were eager to appreciate, acknowledge others’ success, and even look up to them as role models. That signaled how much we have grown as better humans. Bidding goodbye was never easy. So, Divesh suggested we go for a hike. I wished it was on a bike. Alas, everyone wanted a walk and talked. We soon realized that we went very far. The terrain was rugged, and soles of my new cheap shoes were jagged. The rocks were hurting, and my legs were exerting. My pace decelerated. Then came the astounding announcement that rattlesnakes infested the area. I could neither run nor remain placid. I comprehended why Divesh became a proficient runner in this place. He became my instant idol and tried to emulate him, as the fear was gripping and galvanizing. He made bidding goodbye very easy for me. Maybe, our Indian marathon athletes should train here. They would become unbeatable and of course unstoppable.

We then drove to Palm Springs. A major part of the drive was very windy. When we stopped at the rest area, a bemused traveler pointed out at the thousands of windmills installed along the highway, wondering why people waste so much of money just to experience some breeze in the hot desert. His innocent incomprehension was insightful. I almost fainted. He thought it was from the heat!

After we had checked into the resort, we went for some snacks. I noticed a couple sitting outside beneath a beautiful umbrella. The husband had ordered some tea, and the waiter brought him the same. Just then a bird sat on the edge of the umbrella and enjoyed its pleasure of elimination. That fell right into the tea cup, unnoticed by the husband. As the man sipped the newly re-created tea, he was thrilled by the out of the world flavor, and profusely thanked the waiter, who was clueless.
The now hysterical husband went on to compel his innocent-half to savor the new flavor. He went for the server to find out what type of tea it was. In the meantime, the bird wanted to relive its pleasurable expulsion, dropped its droppings into the cup as the wise-half watched. Furious, she beckoned her jubilant-half to convince him to buy the birdie and not the tea from the resort for his ethereal experience of the exquisite bird brew.

If Fate Had A Date!


When you get a premium extra leg room on a long flight, you are instantly reminded of LG’s slogan “Life is good”. Well! I paid a premium to experience that prime feeling. However, when I was about to treasure that short term pleasure, life reminded me that pleasure in any measure will float away like a feather as Casio says “Expect the unexpected”. A baby in the backseat as if rigidly following Harley Davidson’s “American by birth, but Rebel by choice”, felt envious of my pleasure in leisure and began to express it with a deafening cry. The helpless mom trying to be considerate to others, and to herself picked her up, and meandered in to my cherished, premium leg space as if it was Disneyland’s “Happiest place on earth (or was it on flight!).

I had no choice but to retract my reveling limbs from my space of luxury. The cry now transformed into a mocking smirk on that intolerant infant. Mother pleased with her baby’s new found happiness, decided to linger longer than an hour. This procreator, and cry creator combination became an instant inspiration for similar pairs on the flight. One after another began to savor the Shangri-La with my begrudging benevolence. It seemed my premium space had Sony Playstation tag: “live in your world, play in ours”. I was made to sit and feel like McDonald’s “I’m lovin it!”. I could neither evade the charming smile of the child, nor reconcile with the aching recoiled legs of mine. I finally had to settle with Burger King’s way “Have it your Way!”

It was time to enjoy the joy of the ocean. As we landed in the island of seafood, vegetarian like me had sea, but not food! The resort’s chef had personally handpicked several fallen leaves and decoratively arranged them on a tiny plate, sprinkled with drops of dressing. When asked if there was any other variety of vegetarian food, he promised that he would have grass the next day, perhaps coinciding with lawn mowing tonight, I thought. Having had not so yummies in our tummies, we decided to venture out to see the creatures of the coral world. After forfeiting my biweekly paycheck for the entrance fees, we zoomed in to the coral park. We were then asked to buy some honey to feed the lorikeets. We gladly obliged, and eagerly awaited to satiate the culinary needs of this lorikeet. None came. Environmentalist excitedly informed that if they are not hungry, they wouldn’t feed. We waited. Having had only leaves for lunch, the honey was very tempting. Neither birds would have it, nor we could have it. I wished we were standing at the back of the crowd, so that we could cherish the nectar, which was looking like a loaf of meat for a hungry lion. Social constraints lead to extreme self-restraint.

We then proceeded to the turtle feeding. We didn’t realize that they ate carrots, broccoli etc. it seemed they had a better vegetarian menu than ourselves in the resort. I offered with the pretext to feed the not so hungry turtles. However, the staff showed no interest thus depriving me of the option to satisfy my growling stomach. Interestingly, I saw a couple with some enticing vegetarian snack trying to feed the beastly looking Iguanas. Out of sheer curiosity combined with bare necessity, I approached the couple and enquired where did they get it? The naïve couple couldn’t remember the place or the name, much to my chagrin. But, offered the delicious dissert to me. My face gleamed, my appetite beamed until my esteemed couple screamed “please, feel free to feed the iguanas”. I glanced at them as gloom began to bloom, and muttered “of course, thank you”.

As we boarded our long flight back, we couldn’t carry food from the airport, as it was too small to have any vegetarian options. We were happy to see the cheese platter on the flight menu. We asked for platter, and water, and attendant thought it will make us fatter and gave us the latter to shatter the prospect of eating any veggie matter. I asked for peanuts, she explained to me the cost cuts. My initial hope was great, but as fate had its date, I was left without a plate, and continued my hunger state.
